&lt;p&gt;I installed with no Joy. It was in the Add-in Manager but not the Tools menu. I have Vista\VS2008\Office 2007.&lt;/p&gt;
&lt;p&gt;The registry looked the same as the pics but for 2 items.&lt;/p&gt;
&lt;p&gt;The HKEY_CLASSES_ROOT\CLSID\{}\Programmable key was missing.&lt;/p&gt;

&lt;p&gt;I ran regsvr32 vsspellchecker.dll in the install dir and Joy! It is now in the Tools menu and runs. &lt;/p&gt;
&lt;p&gt;Also note that the &amp;quot;Programmable&amp;quot; key that I mentioned was missing in my previous comment is now in the registry.&lt;/p&gt;
&lt;p&gt;Note, that if you try the regsvr32 command in vista it will fail with UAC on, unless you open the command window in Start-Accessories by right clicking on Command Prompt and selecting run as administrator.&lt;/p&gt;
&lt;p&gt;Is there a reason why it doesn't run all the time? &lt;p&gt;Its listed in the Add-in Manager but not in the Tools Menu.&lt;/p&gt;
&lt;p&gt;Going only from the info you show in your Registry pics I see the following differences:&lt;/p&gt;
&lt;p&gt;1 - in Comp\HKEY_CLASSES_ROOT\CLSID\{xxx...xx} I am missing the &amp;quot;Programmable&amp;quot; key.&lt;/p&gt;
&lt;p&gt;2 - in Comp\HKEY_CURRENT_USER\Software\Microsoft\VisualStudio\9.0\AddIns\SpellChecker2008.Connect&lt;/p&gt;
