The SQL Server 2012 Presentation Management Pack was released together with the September 2013 version of the SQL Server Monitoring Pack.
This new MP contains a new dashboard component: The SQL Server 2012 Databases Summary Dashboard. It is a combination of a state widget, a detail widget, an alert widget, a free space widget and a performance tile widget in a single dashboard layout.
It references hence requires both the SQL 2012 (Discovery) Management Pack and Visualization Component Extensions Library Management Pack to be imported as well.

Here is an example of a dashboard, the Sample Logical Disk Summary Dashboard, that displays the list of all monitored Logical Disk instances and their health states, provides detailed information and visualization on performance data collected, using the performance tile widget component defined in the Visualization Component Extensions Library Management Pack.

I also wanted to use this sample dashboard as a book plug for: PowerShell Deep Dives.

Why are you plugging the PowerShell Deep Dives book in the same article as an OpsMgr dashboard you ask ?

With OpsMgr, you can create PowerShell-based performance collection rules and unit monitors for the logical disk instances of your monitored servers.
Hence, on the dashboard, you can rollup the health state of the monitor to the logical disk instance in the state widget, and show the performance charts of the selected object in the performance tile widget based on the data collected by the PowerShell based collection rule!  :)
