Tech Ed Session Demo Code
Paul Ballard gave an excellent session at Tech Ed on Creating and Using Outlook Add-ins in Visual Studio Tools for Office 2005. This did not cover "Cypress" add-ins for Outlook 2007, but rather the add-ins you can create today in Outlook 2003 using VSTO 2005. He's published his demo code on his blog, and you can find it here.
